Real forms — where to actually get them
We link to the official source instead of hosting copies, so you always get the current version — government forms change, and a stale copy would do you more harm than good. Not everything is a downloadable PDF: some processes moved fully online, and some are only available in person. We tell you which, so you don’t waste time hunting for a form that no longer exists.
Labour Law
CCMA Form 7.11 — Referral of a Dispute

Refers an unfair dismissal or unfair labour practice dispute to the CCMA. Must be filed within 30 days of dismissal (90 days for unfair labour practice).
Search "Form 7.11" on ccma.org.za, or ask any CCMA office for a printed copy — they cannot refuse to assist you in completing it.

Requests arbitration after conciliation has failed (issued a certificate of non-resolution). Filed after, not instead of, Form 7.11.

The employer's declaration of an employee's earnings and employment status — required before a UIF claim (unemployment, illness, maternity) can be processed.
Submitted by the employer via the uFiling online portal. If your employer won't submit it, the Department of Employment and Labour can assist directly.

Maintenance complaint

Lodged in person with the maintenance clerk at your nearest magistrates' court — free, no attorney needed, and the clerk assists with the paperwork on the spot.

Rental Housing Tribunal complaint

Complains about a landlord or tenant dispute — deposits, maintenance, unfair practices.
Each province runs its own Tribunal with its own intake process — search "[your province] Rental Housing Tribunal" or ask your provincial Department of Human Settlements.

Small Claims Court summons

Claims up to R20,000 without a lawyer. Requires a 14-day letter of demand first.
There is no standalone downloadable claim form — the clerk of the Small Claims Court prepares the summons with you once you bring your letter of demand and proof it was delivered.
